  • Statement from you that you have a good intention that the disputes applied have been violated and not allowed.
  • Under the penalty of perjury, an explanation from you that the information above is accurate and that you are the copyright owner or authorized to act on behalf of the copyright owner
  • Electronic signature or the actual signature of the copyright holder or those of the person authorized to process copyright on behalf of the copyright owner in filing confirmation of copyright infringement
